ANNETTE PHELPS
Named in honor of the first president of the Ohio Federation
of Women’s Clubs, this scholarship is available to graduating high school
seniors to use in securing education in the teaching or library
professions at an
The student must be sponsored by a GFWC/Ohio Federation of
Women’s Clubs per capita club and must be in the upper third of his or
her graduating class.
REQUIRED MATERIAL
I.
FROM THE SPONSORING CLUB:
1.
A letter from a club officer or club scholarship chairman
giving a character reference, unusual financial obligations, chronic illness or
disabilities within the family, work experience of the applicant, etc.
2.
The completed 2010-2012 Annette Phelps Lincoln Application
Form.
II.
FROM THE HIGH SCHOOL:
1.
An official copy of the student’s transcript.
2.
Class rank and grade point average provided by
counselor. The student must be in the
upper one-third of their class.
3.
Letter of recommendation from counselor, high school principal,
administrator or teacher.
III.
FROM THE APPLICANT:
1.
Copy of letter of acceptance from an

Letter from applicant establishing proof of financial
need, including any financial aid applied for and being received.
3.
Copy of completed GFWC/Ohio Federation of Women’s Club
Supplemental Questionnaire.
4.
Completed and signed application form.
DIRECTIONS
FOR CLUB
·
A club may only sponsor ONE candidate per scholarship per
year.
·
By March 1st, the sponsoring club should collect
all required documents as listed in these instructions and mail the completed
file in ONE envelope to the Chairman of the GFWC/Ohio Federation of Women’s Clubs
Scholarship Board of Trustees (see above listing).
·
Any applications postmarked later than March 1st
will be disqualified.
·
Omission of any required materials will disqualify the
applicant.
·
Please use proper form for the appropriate scholarship.
·
The club should make sure that the applicant as well as the
club representative has signed the application.
CAREFULLY FOLLOW THESE DIRECTIONS

Assemble the nine (9) pieces of required materials listed
above. These nine (9) pieces
constitute a complete file. Any
omission(s) automatically disqualifies the applicant.
2.
Return file to your guidance counselor at a date set by Guidance
Department in conjunction with the sponsoring club.
GENERAL INFORMATION
Immediately following the GFWC Annual Meeting (Convention),
the Chairman of the Scholarship Board of Trustees will notify those students
receiving scholarships as well as the sponsoring clubs. Checks will be mailed by the Treasurer of the
GFWC/Ohio Federation of Women’s Clubs no later than August 1 to those receiving
scholarships.
Applicant must notify the GFWC/Ohio Federation of Women’s
Clubs Scholarship Board of Trustees Chairman by June 30 of decision to
accept/reject the scholarship.
